The latest Herkimer County NYS Health Inspections found numerous critical violations including, rodents, insects, filthy bathrooms and cross contamination.

MAIN MOON BUFFET
40 CENTRAL AVENUE, ILION

[UPDATE] - 9.22.17 - Main Moon Buffet has responded to the reported health code violation with their latest inspection report.

A surprise inspection by the NYS Health Department on August 28, 2017 found the potential for insects, rodents and vermin, potential cross-contamination, hazardous foods not kept at proper temperatures.

  • Water/ice: unsafe, unapproved sources, cross connections.
  • Potentially hazardous foods are not kept at or below 45oF during cold holding, except smoked fish not kept at or below 38oF during cold holding.
  • Potentially hazardous foods are not stored under refrigeration except during necessary preparation or approved precooling procedures (room temperature storage).
  • Food not protected during storage, preparation, display, transportation and service, from potential sources of contamination (e.g., food uncovered, mislabeled, stored on floor, missing or inadequate sneeze guards, food containers double stacked).
  • Non-food contact surfaces of equipment not clean.
  • Effective measures not used to control entrance (rodent-, insect-proof construction). Harborage areas are available for rodents, insects and other vermin.

DOPP'S INN

Inadequate, dirty bathrooms without toilet paper or closing doors, dirty ventilation hoods, and cross-contamination were problems at Dopp's Inn on August 10, 2017.

  • Cooked or prepared foods are subject to cross-contamination from raw foods.;
  • Toxic chemicals are improperly labeled, stored or used so that contamination of food can occur.
  • Food not protected during storage, preparation, display, transportation and service, from potential sources of contamination (e.g., food uncovered, mislabeled, stored on floor, missing or inadequate sneeze guards, food containers double stacked).
  • Non-food contact surfaces of equipment not clean.
  • Toilet facilities inadequate, inconvenient, dirty, in disrepair, toilet paper missing, not self-closing doors, missing hand wash signs.
  • Floors, walls, ceilings, not smooth, properly constructed, in disrepair, dirty surfaces.
  • Lighting and ventilation inadequate, fixtures not shielded, dirty ventilation hoods, ductwork, filters, exhaust fans.

August 17, 2017 inspections find lighting problems and food temperature issues.

  • Accurate thermometers not available or used to evaluate potentially hazardous food temperatures during cooking, cooling, reheating and holding.
  • Lighting and ventilation inadequate, fixtures not shielded, dirty ventilation hoods, ductwork, filters, exhaust fans.

NEW TOKYO HERKIMER
100 NORTH CAROLINE STREET, HERKIMER

Toxic chemicals, improper storage, and unsanitized cleaning rags were a few of the issues discovered by the NYS Health Department at New Tokyo in Herkimer on August 25, 2017.

  • Toxic chemicals are improperly labeled, stored or used so that contamination of food can occur.
  • Food not protected during storage, preparation, display, transportation and service, from potential sources of contamination (e.g., food uncovered, mislabeled, stored on floor, missing or inadequate sneeze guards, food containers double stacked).
  • Wiping cloths dirty, not stored properly in sanitizing solutions.
  • Non-food contact surfaces of equipment not clean.
  • Miscellaneous, Economic Violation, Choking Poster, Training.

These restaurants/clubs/churches have 30 – 60 days to fix the critical problems discovered by the NYS Health Department. You can get all up to date Health Department violations at the New York State Health Department website.
